It’s no secret that the first year of Cardiology fellowship has been difficult for me. I was warned that it would be more challenging than residency, but thought - how bad can it be? I wrote a book during residency, right? Survived the MICU during the first pandemic in a century? I can handle anything, right?

I was wrong. It’s been harder. The toll on my body has been immense. There have been times when I’ve actually thought -“ how am I supposed to sustain a pregnancy through this?” And the answer is : I’m not. A recent study by @drmarthagulati et al entitled, “Childbearing Among Women Cardiologists: The Interface of Experience, Impact, and the Law” shows in no uncertain terms what this field does to #wic #womenincardiology & how many, frankly, illegal practices we have to confront in our efforts to bear children while practicing in a field we gave 14 years of our lives to pursue.

I’ve thought often about quitting fellowship. I haven’t because I love cardiology, and frankly, I’m entitled. I think I should absolutely be able to “have my cake and eat it too.” I don’t think it’s fair that I’m expected to sacrifice my reproductive potential to make a hospital system money, or to avoid inconveniencing already overstretched colleagues. Not when my colleagues who don’t carry pregnancies don’t have to do the same. I believe that if I am recruited into a field, I should be supported in it too. And I think that physicians, who are well aware of the mental & physical health effects of sleep deprivation, long hours, and lack of reprieves, shouldn’t allow each other to put themselves at risk for a “calling”… or a job.

Dr. Gulati’s paper ends with solutions. I hope the Powers That Be are paying attention.

Know that whether or not you match says nothing about your capabilities as a physician- some of the best doctors I know did not match on Match day. Hold your head up!

Writing is my second job. Like medicine, traditional publishing is an opaque, confusing industry, and navigating it often felt like walking blindfolded through a maze. Thank goodness for my writing big sis @rissneilson (buy DEEP IN PROVIDENCE) or I would be constantly lost.

One of the scariest parts in the process was asking for blurbs to go on the book’s jacket. Essentially, I have to ask authors I have never met to spend their precious free time, time they could be using to, well, write more, to read my book and then write something it, for free. Because I said “pretty please.” Oh, and they have to do it on a deadline. I’m a complete newcomer into the writing world, & I’m not able to attend the conferences, panels, or be active in the slack groups where I could make potential connections because, well, my 1st job is mad demanding, so I often feel like the kid in the back of class no one talks to 😅😂. Needless to say, I was really nervous that no one was going to want to blurb my book.

But imagine my surprise / delight when not one, not two, but SIX authors I deeply admire actually answered my team’s calls! I’m still beside myself, and so very grateful. Sometimes, when I’m having a bad day, I pull up the doc with their blurbs just to lift my spirits.

I’m also so thankful for the authors who’ve opened up lines of communication to give me HOURS worth of guidance (esp you, @brendajanowitzwriter 💍) and shared my book on their feeds (I’m still flipping out about @alihazelwood 🥺!)

T-4 months until y’all can finally read ON ROTATION!

This is going viral on Twitter, so let me post it before somebody else does 😂- I think medicine, and male dominated specialties in particular, loves to valorize women / people who “do it all”- who hunch over surgical tables with 40week bellies, who are constantly available to take on more patients, more cases, more committees. I’m all for personal choice, but so often it feels like self sacrifice is considered a part of the gig.

Next month is Women’s Month, and instead of highlighting the women who give their entire lives to medicine, I would love to identify and highlight the ones who have prioritized life outside of healthcare and found balance. More info… when I’m done with consults.

Before I became a Cardiology fellow, I was an Internal Medicine resident with a primary care panel in South side Chicago. One cornerstone of my practice was screening for lung cancer, and in my three short years, many of my favorite patients were diagnosed with lung cancer. Lung cancer remains the #1 cancer killer among Americans, with Black Americans consistently having worse outcomes. While the last decade has seen incredible advancements in available therapies, Black Americans are still underrepresented in clinical trials (making up only 3.1% of participants.)

Recruiting a diverse set of research participants (by age, ethnicity, race, gender, etc) is important, because different therapies affect patients in different ways. Thus, representation in these trials leads to better, more fine-tuned therapies, better awareness of side effect profiles, and more information for clinicians to provide future patients. In addition, enrolling gives patients access to new, cutting edge treatments.

“1. I went into pediatrics of course for my love of kids - they are just so honest and so fun to talk to and you can’t really get mad at them for getting sick or not listening to you lol. Also I love having to know physiology from newborns to even adults especially for our patients who are adults who have congenital heart disease palliations or pediatric cancers.

  1. There are a fair amount of procedures from lines, to chest tubes, to intubations, etc. and I actually do love rounding and going into deep dives into physiology.

  2. Even though I find all the systems fascinating I was not interested in being a specialist of one organ system - I like being in the middle and asking my colleagues who are experts in those fields for help.”

Thanks for the wonderful insight, James! Designing PICU boy was tough- he has so many elements that are seen in other specialties! In keeping with that, I gave him one of the same weapons as Anesthesia girl (laryngoscope blade scythe) & the bronchi motif seen in IM & Pulm Crit girls. To highlight his Peds element, I gave him a spell circle that’s has vaccination runes, and added simple shapes to his cloak. I wanted him to be flashy but stylish!

@thestyleprescription is one of my long time Instagram homies, a memelord with a heart of gold and a style so fresh that people keep trying to steal his identity to catfish girls (not even joking, & it’s only funny when it’s not happening.) So when he told me he’d like a PICU boy to celebrate his match into Pediatric Intensive Care, you know I had to let him jump the queue!

I don’t know much about PICU as I didn’t get exposed to it in medical school, & so this design was a bit more challenging, but James arose to the challenge! I love PICU Boy. Whenever Peds decides to commission, they’ll definitely be an adorable Mage to stick with the theme!

Whenever I get a little more free time, I find myself powering down. I work less, draw and write more. Reconnect with my friends. I read. All of these things have a paradoxical effect of both nourishing me and making me anxious. I definitely tie my self worth to my productivity, and I consciously have to turn off a part of my brain that expects me to be able to do more. I’m working on breaking these unhealthy thought patterns by forcing myself to live in these moments of enjoyment.

One nice thing about the whole 2.5 career gig I’ve got going is that I can double dip on work & pleasure. Reading is part of my writing career now, & so lately I’ve been getting lost in books. Here I am surrounded by some of the books I’m currently reading/ about to read. What’s on your TBR?
